Preparing for Your Visit

​Before the visit:

• Write down any questions you may have for me.

• Prior to the visit, I’ll send you some forms including a consent form, and a health history form. Please fill these out beforehand so as not to interrupt our consult time.

• If you are using a breast pump or nipple shield, please bring them with you, or have them nearby. This will allow me to demonstrate use, assembly and proper use. 

• During a home visit, please place pets in another room. Animals can become very possessive and territorial with babies and can affect our consultation time. 

• Please have baby with you for oral exam, latch assessment, pre-feed and post-feed weights, and any other concerns I can address.

After the visit:

• I will provide you a care plan and advise you how to proceed for optimal success.

• I will fax a written report to your child’s Pediatrician, OB/GYN, and/or other health care providers involved in your care.
