Preparing for Your Visit
Before the visit:
Write down any questions you may have for me.
Prior to the visit, I’ll send you some forms including a consent form, and a health history form. Please fill these out beforehand so as not to interrupt our consult time.
If you are using a breast pump or nipple shield, please bring them with you, or have them nearby. This will allow me to demonstrate use, assembly and proper use.
During a home visit, please place pets in another room. Animals can become very possessive and territorial with babies and can affect our consultation time.
Please have baby with you for oral exam, latch assessment, pre-feed and post-feed weights, and any other concerns I can address.
After the visit:
I will provide you a care plan and advise you how to proceed for optimal success.
I will fax a written report to your child’s Pediatrician, OB/GYN, and/or other health care providers involved in your care.
